Just Cause eviction disclosure

Hello all,

I am a new landlord in California and have a tenant moving in next week. I am providing all legal documents to them an am unable to find the "Just Cause" Eviction Disclosure (I believe I need JC-2). Can anyone provide them for me? 

If I plan to create a trust or LLC in the near future, should they be provided with JC-1 or JC-2 as the trust/LLC is not set up yet? Any help is appreciated!
