Tenant Repair Payment Request Template

Hello Everyone,

I just had a main drain backup. When the plumbers came out to clear the drain, they recovered wipes from the toilet drain, which was causing the blockage to the main drain. The Tenant is claiming he doesn’t use wipes, even though the items recovered from the cleaning are a bunch of wipes directly from his unit’s toilet.

Does anyone have a template I can use to request payment from the tenant for the bills incurred for the drain cleaning?

My lease has a clause stating that the tenant will be responsible for costs if the plumbing issues are due to tenant’s negligence whether accidental or purposeful.

Anyone who has dealt with situations like this with a tenant: your input is greatly appreciated.

For further context: it’s a duplex and his unit is the second floor. First floor is vacant.
