Midterm Addendum Needed

Hello everyone, I found a great location for my midterm rental. Nothing in the lease has anything about subleasing ir listing in airbnb websites or etc. I know I need to add an addendum about hosting to midterm travelers. Can someone point me in the right direction on where I can get the form to add an addendum into my lease about what I'm using the corporate unit for.

You would probably send it with the application, that way everything is up front with no surprises.
