Operating agreement form anyone?

I'm a newer investor and am working on a hold deal with a friend. I would like to protect the agreement and relationship by creating an operating agreement and getting it notarized. I'm looking for others who have done this with success and how they laid out the agreement. and specifics to add or things you wish you would have done differently. Thanks for any feedback.
