SOW (Scope of Work) Rehab Template

I'm trying to get more organized and systemized, specifically when dealing with contractors and getting quotes.

What I'm looking to do is to have an SOW Template for assessing a rehab.  This would be a pre-printed form that I could use to walk through a property and document all the rehab items to do.  It would have all the major components listed out (roof, bathroom, kitchen, HVAC, etc.) and for each component a few common tasks (roof sheathing, roof maintenance, replace roof, etc.) for each component.  And then I would just check off the tasks to be done and add some notes.

Then I could type that up and hand that off to the contractors for the quotes.  That way it's all there, in writing, exactly the same SOW for each contractor.

I'm sure others have done this, can anybody share a template that I might be able to start with? Don't want to reinvent the wheel...

When I first do a walk through on a property I use this while I walk the property with my camera phone in hand to record the whole place top to bottom narrating the whole thing.  

Then when I get back to the office I re-watch the video over and over again and enter the spreadsheet data into my favorite software to do a full on rehab estimate.

Both the punch-list and video are necessary for me.  The punch-list so that I don't forget to check everything while I'm there and excited about the potential, and the video for when I did forget something after having left the place.  Been doing it that way for years.
