Tenant Lease Extension Process

Question for property owners/managers familiar with the cook county rules:

I have a tenant that is approaching the 60 days till end of lease and I was wondering what the rules require for resigning a lease and even how to ask for the tenant’s desire to resign. Are there Cook County specific forms and to resign, do they have to resign the Cook County lease or will a simple extension form work as an addendum?
