House Information Sheet for property data (appliance models, etc)

Does anyone have any type of House Information Sheet, that could be used to put all the information for a property in one place so I could find it quickly when repairs or maintenance is needed.

For example, when I scheduled an appliance repair, I don't want to have to go searching for a photo of the appliance to read what model it is or hunting in a physical file to find the receipt.  I could also put any data I've collected, such as the measurements so I know what will fit in the space when I have to replace an item again.  Room measurements could also be kept so when a prospective tenant has questions, the information would be easy to find.  Warranty dates of expiration or other time saving info could be kept as well.

Does anyone have anything like this they could share, perhaps an excel sheet or a word doc set up?

We have something like this as well. We use a plain Excel spreadsheet with all appliances model numbers, HVAC models, hot water heater models/make/manuf. date., furnace filter sizes. We also have a separate sheet for each property with community name, amenities included, mailbox number/locations, which utilities are included (if any), parking space numbers, trash days, etc. They do prove useful for sure.
