Best way to notify tenant? - 30 day notice termination

Hi all,
I've been waiting on attornies to consider my project-bid -- on another platform Bigger Pockets recommended (and still no replies!).... so am posting here for additional insight in the meantime.

We have a tenant on a Month to month lease in Ohio.

He is consistently late on rent. 

We have a clause that allows for a 30 day notice to tenant to vacate, but have these questions:
a. Besides emailing him a 30 day notice, does OH require a reason? We have seen both "yes" and "no, no reason is necessary" by OH legal websites.

b. What kind of verbiage should we consider to ensure he  knows he is still responsible for the rent (for the 30 days), the late fees ($30 per day), and will be responsible for any damage done to the home should he do so before leaving.
