I think if you ask 100 people you'll get 100 different answers on this one.  I have used several different banks over time but no one particular has my opinion of being the best or one that I love.  I hate the house buying process (retrieving documents, explanations, wasted time) so I would recommend using a loan agent that you like and you feel comfortable with because there's going to be a ton of communication during the process.

It depends on your experience level.  I would recommend you improving you credit score if you're relatively new to investing.  You certainly can educate yourself on Bigger Pockets in the meantime.  

If you've got some miles under your belt I would say investigate the offer from CityWorth Mortgage.  They may have received your info from the Bank that you'd applied with when they knew they couldn't do your loan.

Check with your realtor or local bank.  The Realtor will know people who he/she can refer you to based on their past experience(s).    If you don't have a Realtor yet, check your local Real Estate Investors Club or if you haven't purchased a home before and you're renting, check with your property management for a recommendation.

I just set one up with Legal Zoom.  I think if its just you in it then straightforward Legal Zoom will be fine.  When you get into partners and who get what if someone dies then you may need more expert advice.  Changes can be made down the line if needed at a price.

The thing with Legal Zoom is their experience.  They've done LLCs in every state and every situation.  Not advocating them, but I had a similar question two weeks ago when I started.
