In terms of rent : price, you can still find plenty of properties that will fit the bill for the "1% rule" if that is what you are referring to. If you are looking for properties with minimal repairs needed you will want to look in the NW, NC, and NE parts of town, typically outside of Loop 410. However, the Downtown areas are where a lot of the action is happening. Tons of new builds, flips, and large commercial projects going on. These are older homes, requiring larger rehabs and more risk. Downtown is a great area for Short-term rentals if that is a strategy you are interested in. Foundation is a common issue with San Antonio properties. The city has also been cracking down on proper permitting recently, so definitely go by the book and do not cut corners when it comes to that.

With 22k I would recommend getting in touch with a few Hard Money Lenders, Private Lenders, or forming partnerships with people and leverage each other. You can do this by attending meetups in San Antonio.
Buying a home all cash does not make much sense to me unless you have a ton of it. For one, you will be married to that house and that house only until you sell it. What happens if something goes wrong and all your cash is tied up? Second, the opportunity cost of waiting around until you have 200k is tremendous. You won't be able to do a single deal for years if you are on a fixed income.
Debt is not a bad thing when using it to make money. If you'd like, I can put you in touch with a few Hard Money Lenders I deal with daily.
Have you thought about doing a few wholesale deals to gain quick capital, then move into flipping / rentals?

Foundation shifting is pretty common for San Antonio. If the numbers check out to your desired criteria and you have done your due diligence it sounds like a buy to me. If you are worried about the foundation in this house, have it looked at by a pro. I wouldn't concern myself with other homes that have issues if you are not looking at those other homes to buy. You are looking at this one. I have a reputable foundation contact I would be happy to share with you if interested.
