what kind of nurse makes 40 to 50 an hr?
assign yourself as the representative. if you have a previous llc use that as a template. I had a lawyer make me a business in iowa a few years ago. cost 750, plus the stage fee of 75ish. formed a new llc this month with Iowa and magically had the paperwork the lawyer did so I used that ad a template. it was maybe a half page with a bunch of confusing here as there in forth wording
