The fence was leaning all over the place, there was garbage filled 4 ft deep in the air vents, and normal maintenance on almost anything was never done.The neighbors next door were disliked by the whole neighborhood for causing problems.

I change the filter (custom merv 5) every month, tenant says unless it's set to low 60s can't sleep well, air is stuffy. 

Since you're not planning to remove the stair or lay down carpet, your focus should be on closing off air gaps, adding dense materials to block airborne sound, and absorbing reverberations inside the stairwell.One of the most effective ways to reduce airborne sound is to add mass and sealing to the underside of the staircase.

If you have attic access and the HVAC has the capacity to push the air, then you add an additional duct.
