How do you handle additions to the original structure?

I had a question arise which I was not completely confident in knowing the answer. When dealing with a home that you want to add square footage to, what are some of the challenges that arise with permitting, issues with the city, etc. I have heard of some horror stories. What would be the smoothest method of going about this?
