Revocable trusts -please help ASAP

Hi guys,

So I am leaving out of country in a few days. Will be flying over the war zone, really worried that something may happen and I end up dead.

My attorney who is doing the trust is taking his sweet time. Its been a couple of months and the trust is not done yet.

I doubt he will get it done before my flight...

At this point I am thinking I will do it myself... I have printed a revocable trust form from the internet and will do  it myself. 

When I come back I will deal with the attorney, may be he will disolve my trust and will create a new one..... Now, I just dont have the time...

So, I will fill out the form, then what? Do I notarize it? Do I record it in my county at the assessors office? What do I do after I fill it out?
