How to get a house out of a trust?

Asking for a friend; this is all the info I have, but I can request more from him based on your feedack:

The family has a house in a trust. The executor of the trust is my friend's mom. It formerly belonged to their deceased aunt.

There is a medicare lien out there, but they aren't sure if the lean is against the trust or the aunt? When their aunt died, would that lien have been moved into the trust? 

The house needs $30k of work or demo'd, including a septic tank that needs pulled, so it has no value.
