Looking for Notary Form Letter for Vacant Property-

Hey BP- I need to submit a notarized letter to the city of Oakland stating my property has been vacant for more than 60 days. Does anyone have a form letter they've used for this purpose and are willing to share? If not do you know what's required? I've scoured the internet and can't find a similar letter. Thanks, Tim
