Whether or not to raise rents depends on the market and whether you are happy with your tenant and should have nothing to do with a rule of thumb yearly increase.
Tenant turnover is one of the highest costs to a landlord. If you are happy with your tenant the goal is to earn the most rent without charging so much the tenant decides to move elsewhere. If your new tenant is a bad one, you can easily lose what you earn in extra rent.
I would not suggest an increase unless the market rent in your area has gone up significantly in the past year –If so, I would suggest increasing rent less than the market area increase since you are already on the high-end (I'm assuming this is because you have a property better than average) and you wish to keep your tenant.

As a property manager I would agree that all three are excessive. Either a pet fee or a pet deposit + pet rent are more common in Colorado.

I would suggest being very deliberate when writing your descriptions. Right the description for the type of person that you want in the property and for the type of person that is a good fit for the unit. Also pictures, pictures, pictures. Have lots of pictures and consider having them professionally done. Good pictures are the key to attracting renters. Price the rent appropriately – It will take longer to rent if pricing is above market rents; however, a renter that is able to pay more is usually a better renter. I use postlets to for listing and I don’t post on craigslist (too many scams).

First off, I always recommend referring to the lease in situations like this. Second, for your own protection and wallet I would NOT leave this up to your tenant to fix. Yes, it's a pain; however, you protect the value of the property by ensuring it's well maintained. For a small price, you can prevent additional maintenance down the line and keep your tenant happy. Tenants who feel cared for take better care of your property. It’s a good thing that your tenant will report plumbing issues. Happy tenants = less turnover (the most expensive cost to a landlord) and properties in better condition.
