Tenant let boyfriend live in house w/o my consent, against lease.

HI all, Im sorry if this has been posted in the past.  This is my first day on the site!  So I have a tenant who has allowed her boyfriend to live with her without my consent.  A lawyer friend says i need to send  a letter referencing the section of the lease that states the number of occupants.  So one question is, are there form letters for this type of thing?  And if anyone has other advise feel free to give it.  Additional info, i ran an app for him because she was saying he was moving in when the new lease went into effect and there's no way I would ever approve him.  He's got really bad credit (not the end of the world) and he has an eviction.

Don't worry about professional templates or fancy legalese. Keep it short, simple, and professional.

"Dear Tenant,

I recently learned you allowed your boyfriend, John Doe, to move into the unit without my knowledge or permission. This is a violation of your lease. I require all occupants to be screened, approved, and on the lease before occupying the property.

I will not renew your lease unless your boyfriend is willing to apply, pass my screening, and sign the new lease."

Or you could say, "I will not be renewing your lease due to this violation. Your lease will terminate on May 31st, 2018."
