I was never asked these questions by my insurance company when I changed my primary residence to a rental. I just had it changed to a business owner policy, they asked a few questions about what date etc, and that was it.

The first thing to do is to determine what the rents are for the units. Then using the numbers you fill out the Bigger Pockets rental calculator to determine if this is even a good deal. Just because you can buy it doesn't mean you should. Good luck. I hope the numbers work out for you.

One option would be to provide a gift certificate of some kind. So it's not acknowledging that the unit was dirty, but that you appreciate them as a new tenant.

You are legally bound by the current lease until it expires. You can try and have the tenant sign a new lease that prohibits smoking but they would not be required to sign it.

Check you own market for what is commonly included. You don't want to be the unicorn. In my area it is customary to include the refrigerator so I do. If it wasn't customary I would not.

As you already aren't sure that she is the type of tenant that you want in your building you should just raise rent to market rate. If she leaves then consider that an opportunity to get the type of tenant you would like in that unit.

Make sure that you are not rejecting them because they have a child. That is illegal. You should have set acceptance criteria that you follow. If they meet it and you reject them you are opening up yourself to trouble. If you can reject them based on your rental requirements you are in the clear.
