 I've seen a lot of these policies and they're often basic perils and actual cash value only.  Be careful.

 Foremost typically does exterior inspections after purchase. Are you sure they didn't visit the properties? Most inspection companies don't contact the property owner unless necessary.

This is a very common practice and it would be much more rare for an insurance company to not inspect a property after signing a contract.

 I wouldn't be overly concerned that it's surplus lines carrier but I'd be VERY concerned that the agent didn't mention it.  Beware of call centers!

 The cost won't necessarily go up from your current policy.  It could, every situation is a bit different.  The price could be affected by MTR vs. LTR but mostly in what policies/carriers are available to you rather than a specific surcharge as I mentioned in my reply above.

 Cost could change but the bigger issue is making sure the usage is still eligible for the type of policy you have.

As an example, at the time of application, a company might ask are all lease agreements for one year or longer.  If the answer down the road changes and you don't notify your insurance provider that could cause an issue if there's a claim.

 The only way to answer this would be to review your actual quote/policy.  Coverage can vary dramatically even within the same insurance company.  

In terms of your grandfather living there as long as he's not the owner having a dwelling fire/rental property insurance policy is appropriate.

As @Greg Scott pointed out he should have a renters insurance policy whether or not he's actually paying rent.

In terms of liability coverage, that depends on what you have to protect so there's no one-size fits all approach.
