I hate PMI like you do and have never paid it; BUT you have to crunch the numbers and if you work hard and get the best deals; you can get the numbers to work to purchase even with PMI. I can't stand paying PMI so I would keep looking for a better deal that you have to pay PMI and you are still positive cash flow OR find a deal that you put more down to avoid it.

As everyone said; NEVER EVER pay more than the work that has been completed; but you now already know that. The best thing is to keep calling; harassing gently; write letters; find their home phone number etc.; contact BBB; Bad reviews on Angies list; Yelp; Google reviews; just try to be a pain in the butt; call TV stations to see if they take your story etc. If they will not finish the work then see if they will give any $$$ back. You can also file a small claims court case I would imagine. 

Bottom line keep trying to see if you can get them to finish but let them know what you are going to do if they don't.

Not sure anymore on the long thread which appliance (refrigerator vs. dishwasher) but bottom line as a NJ landlord; fix any/all appliances you just inherited by buying the property; but also get them to sign a new lease before/as you are getting the appliances fixed. You need to do the right thing and fix them. After that if you want to put in your new lease they are responsible to fix in the future; that's up to you.

Try the area rugs if that makes sense unless the tile is really old looking. If this is a long term rental for you (hold the property 5+ years) then I would bite the bullet and replace it with what you want. You could find somebody pretty cheap to demo it in 1-2 days (not that much money); than you could put down whatever you wish. If your not going to hold onto the property long than do the rugs or something else. Be careful going over tile with something other than carpet (I hate carpet) as it will show all the grout lines/seam etc. unless you put an underlayment on top first.

Ceramic Tile; Allure; or Hardwood floors is the way to go. NEVER CARPET unless you like throwing your money away.

Curious if anyone has bought this system from Joe McCall (Real Estate Mastery). Its not expensive but is it all hype ? Anyone use this ?
